New Patients: You will receive an activation link from your Patience Service Coordinator to create an account.

First-Time Appointments

Before your first visit, we encourage you to reach out to your Patient Service Coordinator (PSCs). Our PSCs are skilled and knowledgeable liaisons who can answer any questions you may have. They can walk you through the process for your first appointment, give a checklist for things to do beforehand, and provide guidance for what you can expect.

Interested In Becoming a Patient?

We want to make sure every GPM patient is given the full time and attention needed to receive quality care, which means our physicians often have limited availability. However, we evaluate our caseload on a regular basis and encourage you to request being added to our waitlist.

Our waitlist is handled through MDVIP. You can call 1-888-696-3847 to be placed on a list and receive notifications about openings with one of our doctors. We thank you for your interest and patience.
