MDVIP Annual Wellness Physical (AWP)

Scheduling

  1. Please call the office to schedule your MDVIP Annual Wellness Physical (AWP) 1 year after your last Annual Physical or Medicare Annual Wellness Visit.
  2. There are 2 parts to the MDVIP Annual Wellness Physical. First is the pre-physical testing at our office. Second is the Annual Wellness Visit with your physician that will take place, approximately, 2 weeks later.
  3. Please allow 1 hour for the pre-physical testing visit, and then an additional 1.5 hours for the Annual Wellness Physical visit with your physician.

Part 1 of the AWP

PRE-PHYSICAL

Part 1 (pre-physical) testing will be performed by one of our MDVIP Medical Assistants- Suzanne, Shardae, or Leslie. Please fast for 8 hours prior to coming into the office for this visit, unless you have a medical condition that makes fasting unsafe. Water, black coffee, and tea the morning of the visit is fine. Please take all of your regular medications.

When you arrive for your pre-physical testing, please check-in with one of the patient service coordinators (PSCs).

If you do not wish to receive any one of the specific tests included in the pre-physical testing, you may opt out.  However, we encourage you to try all of the components.

The pre-physical testing will include the following:

  • Bloodwork:
    • Cleveland Heart Labs (CHL) will include a detailed cholesterol panel, specialized inflammatory markers, hemoglobin A1c, TSH (evaluating thyroid function), uric acid, and vitamin D level. We will also be sending a CBC (complete blood count), CMP (electrolytes, kidney, and liver function), and free T4 (also evaluating thyroid function). Urinalysis for patients 50 and over. PSA for men 50 and over.  Hepatitis C and Hepatitis B screening will be done one-time on all patients.
  • EKG
  • In-Body Scale (measuring muscle/fat ratio and distribution)
  • ABIs (ankle-brachial index) – blood pressures taken in arms and legs, screening for peripheral vascular disease
  • Vision Screen
  • Hearing Screen
  • Grip Strength
  • Gait Speed
  • Spirometry (currently not available)
  • Questionaires

Part 2 of the AWP

On the day of your 1.5 hour visit with your physician, there will be 2 components to the visit. Of note, you do NOT need to fast beforehand.

First, your physician will review your medical and surgical history, medications and allergies, social and family history, immunizations, and preventive health screenings like colonoscopies, mammograms, etc. This component will be billed to your insurance.

Second will be the MDVIP Annual Wellness Physical. This will include a complete physical exam, in-depth review of the pre-AWP testing results, as well as counseling related to those results. This component, as well as the pre-AWP testing, is included in your MDVIP membership fee.
